Zepbound comes in three forms: single-dose vials, the multi-dose KwikPen and a single-dose pen. The self-pay prices below apply to the vials and the KwikPen. If you’d rather skip needles, two GLP-1 pills are now approved as well.
Where Can You Buy GLP-1 Injections Online?
You can buy GLP-1 injections online from the makers’ own pharmacies, from online and retail pharmacies, or through your insurance’s mail-order pharmacy. The prices are mostly the same, so the choice comes down to delivery, refills and payment.
| Where | What it sells | Cash price per month | Good to know |
|---|---|---|---|
| LillyDirect Pharmacy | Zepbound vials and KwikPen | $299 (2.5 mg), $399 (5 mg), $449 (7.5–15 mg) | $449 needs a refill within 45 days, otherwise $499 to $699; home delivery or pickup at Walmart |
| Amazon Pharmacy | Zepbound KwikPen | From $299 | Also fills $50 Medicare GLP-1 Bridge prescriptions |
| Retail pharmacies | Zepbound KwikPen | Same as LillyDirect with Lilly’s self-pay savings card | The card allows up to 11 fills a year and ends December 31, 2026 |
| NovoCare Pharmacy | Wegovy pens | $199 for the first two fills, then $349; $399 for Wegovy HD | Intro price for new patients through December 31, 2026 |
| Local pharmacies with Novo’s offer or a TrumpRx coupon | Wegovy pens | Same as NovoCare | Self-pay only; doesn’t count toward an insurance deductible |
| Costco (members) | Wegovy and Ozempic | Launched at $499 in October 2025; confirm the current price | Needs a Costco membership and a prescription |
| Your insurer’s mail-order pharmacy | Whatever your plan covers | Your plan’s copay | Usually needs prior authorization |

Watch the refill window on Zepbound. Miss the 45-day window and a 10 mg or higher box costs $699 instead of $449, so one late refill can add $250.
How Do You Buy GLP-1 Injections Online, Step by Step?
You need a prescription first. After that, buying GLP-1 injections online works much like any other mail-order prescription.
- Get a prescription from your own doctor or a telehealth provider licensed in your state.
- Ask your prescriber to send it to the pharmacy you picked. Lilly and Novo both tell patients to have the prescriber send it to LillyDirect Pharmacy or NovoCare Pharmacy.
- Pay the pharmacy. Use your insurance card if your plan covers the drug, or a savings offer or cash if it doesn’t.
- Get the box. Injections ship cold, and a month means four weekly doses (28 days).
- Buy supplies. Pen needles for the KwikPen and syringes for vials cost extra. LillyDirect adds pen needles to your cart for a fee.
- Set a refill reminder. Zepbound’s lower prices at 7.5 mg and up depend on refilling within 45 days.
Your prescriber sets your starting dose and when to step up. Both drugs follow a label schedule that starts low and rises about every four weeks.
What If You Don’t Have a Prescriber?
A telehealth provider can evaluate you and send the prescription to the pharmacy. The makers even publish lists of independent telehealth companies.
LillyDirect lists five weight-care options with their own fees, which exclude the drug. NovoCare lists eight telehealth companies for Wegovy. Both companies say they don’t endorse these providers or earn referral fees from them.
| Option | Listed by | What you pay for care | Notes |
|---|---|---|---|
| General Medicine | LillyDirect | $25 per visit on average with insurance; $100 self-pay | Takes commercial insurance, Medicare and Medicaid |
| Counsel Health | LillyDirect | $29 per visit self-pay | Or $25 a month on an annual plan |
| Sesame | LillyDirect and NovoCare | $99 a month, or $59 a month on an annual plan | Drug billed separately |
| WeightWatchers | LillyDirect and NovoCare | $149 a month, or $74 a month on an annual plan | Also sells Novo’s multi-month Wegovy subscription |
| Knownwell | LillyDirect and NovoCare | $31 per visit on average with insurance; $299 self-pay | Takes commercial insurance and Medicare |
| AgelessRx | Our review (4.8) | $65 monthly membership | Wegovy and Zepbound at the makers’ prices |
| Found | Our review (3.8) | $49 to $149 per 28 days, depending on plan and insurance | Insurance help; Medicare GLP-1 Bridge support |
Fees for LillyDirect’s options are rates the providers report to Lilly. NovoCare’s list also includes 9amHealth, EMed, Form Health, LifeMD and Ro.
Novo also sells multi-month Wegovy subscriptions through Ro, WeightWatchers and LifeMD: $329 a month on a 3-month plan, $299 on 6 months and $249 on 12 months. Ask whether the telehealth company adds its own fee on top.
Want brand-name Wegovy or Zepbound through telehealth?
How Much Do GLP-1 Injections Cost When You Buy Online?
Most people pay $349 to $449 a month once they reach a maintenance dose, before any telehealth fees. Starting doses cost less, and the price rises as the dose goes up.
Weekly shots come in 28-day boxes, so a year takes 13 fills. A first year of Zepbound at 10 to 15 mg comes to about $5,637 at LillyDirect prices, and about $5,087 if you stay on 5 mg. A first year of Wegovy for a new patient comes to about $4,237, or $2,988 on Novo’s 12-month subscription.
List prices are much higher than these cash prices: $1,349.02 for Wegovy and $1,087 for Zepbound, according to TrumpRx.gov. Always ask for the self-pay price by name.
Which Option Is Cheapest for Your Dose?
The cheapest place to buy GLP-1 injections online depends on the drug, the dose and how long you plan to stay on it. Here’s how the September 2026 prices play out:
- Starting Wegovy: NovoCare’s new-patient price of $199 covers your first two fills of the 0.25 mg and 0.5 mg doses, through December 31, 2026. Local pharmacies with Novo’s offer charge the same.
- Staying on Wegovy for a year: Novo’s 12-month subscription brings the pen to $249 a month through Ro, WeightWatchers or LifeMD, if you’re sure you’ll stay. Add any fee the telehealth company charges.
- Starting Zepbound: the 2.5 mg dose costs $299 at LillyDirect and Amazon Pharmacy. Vials and the KwikPen cost the same, so choose by what’s easier for you to use.
- Zepbound at 7.5 mg and up: $449 a month if you refill within 45 days of your last delivery, or $499 to $699 if you’re late.
- Covered by commercial insurance: ask your prescriber to check prior authorization first, because a savings card with coverage can cost far less than any cash price.
Price isn’t the only factor. Your prescriber decides which drug and dose fit you, and switching drugs to save money is a medical decision, not a checkout choice.
What If Your Order Arrives Warm or Damaged?
Don’t use it. Contact the pharmacy right away, keep the box and packaging, and ask for a replacement. The FDA says not to use an injectable GLP-1 that arrives warm or poorly refrigerated. If you think a product was fake or caused a problem, you can report it to the FDA’s MedWatch program.
How Do You Spot a Fake or Illegal GLP-1 Seller?
If a site will sell you a GLP-1 without a prescription, it’s illegal. The National Association of Boards of Pharmacy says nearly 95% of websites offering prescription-only drugs online operate illegally.
Before you pay, run three quick checks:
- Look the company up on LegitScript and in NABP’s Safe Site search. A .pharmacy web address means an NABP-accredited pharmacy.
- Find the pharmacy in your state’s license database. The FDA links every state board on its state-licensed pharmacy page.
- Search the company’s name in the FDA’s warning-letter database.
When the box arrives, check it:
- It should be cold. The FDA says not to use an injectable GLP-1 that arrives warm or poorly refrigerated.
- Brand-name pens and vials should come sealed in the maker’s packaging, with instructions.
- The pharmacy name on the label should match a real, licensed pharmacy. The FDA has found fake products with labels naming pharmacies that don’t exist.
Skip anything sold “for research purposes” or “not for human consumption.” The FDA has warned sellers of these products, and there’s no prescriber or licensed pharmacy behind them.
Can You Use Insurance or Medicare When You Buy Online?
Yes, if your plan covers the drug. Many plans still don’t cover GLP-1s for weight loss, and savings cards work only with commercial insurance.
With coverage, Lilly’s savings card can bring the Zepbound single-dose pen down to as little as $25, within the card’s limits. Without coverage, the card price for the Zepbound single-dose pen is as little as $499. Savings cards exclude Medicare, Medicaid, TRICARE and the VA.
On Medicare, the Medicare GLP-1 Bridge lets eligible Part D members get Wegovy or the Zepbound KwikPen for $50 per fill through December 31, 2027. Amazon Pharmacy and LillyDirect fill Bridge prescriptions, and your prescriber has to file a prior authorization.
What About Compounded GLP-1 Injections Sold Online?
Compounded semaglutide and tirzepatide aren’t FDA-approved, and the rules for selling them have tightened. They’re cheaper, but they’re not the same as the brand-name drugs.
Mass-made copies lost their legal basis in 2025, after the FDA declared both shortages over. Today a 503A pharmacy can compound for one patient when a prescriber documents a medical reason. The FDA has also sent warning letters to telehealth companies over misleading marketing, including SkinnyRx and Strut Health in February 2026 and Eden in June 2026.
